5) Employment letter on letterhead, plus wage slip copy and  


   W-2 copy (not sure if obligatory) 




6) I-693 Medical Form to be unopened 




7) Form 9003 - tax related. entitled �dditional Questions to  


   Completed by ALL Applicants for Permanent Residence in the    


   United States ?nbsp;and relates to tax info with 4 simple yes/no  


   questions. 




8) copy of current I-94 card 




9) copy of all pages of your passport, including blanks 




10) copies of previous authorisation cards and approval notices (H-1 etc.) 




11) brief summary of your history in US - to make sure you weren� illegal at any point 




12) Birth Certificate or equivalent 




13) Last but not least the Fees!

    I-765 (EAD) : $100
